Important Information Regarding Enrollment Documentation
Please note: If requested enrollment documentation is not provided, the Plan will not pay claims until the documentation is received. Delayed enrollment documentation may result in the denial of claims. In no event will the Plan pay claims submitted or completed more than 12 months after the incurred date.
Before claims will be processed, a Participant must complete and return an enrollment form to the Administration Office and include any dependents the Participant wishes to cover. The Administration Office may require enrollment documentation to verify dependent status, for example, a copy of a marriage certificate, birth certificate, adoption papers or divorce decree. Dependents must also be enrolled, and any requested documentation returned to the Administration Office prior to claims being processed.
Please submit your Enrollment Form as soon as feasible to begin the enrollment process of any eligible dependents. Supporting enrollment documentation will be requested by the Administration Office after the Enrollment Form has been received.
Enrollment forms are available on the Plan’s website (Click here for enrollment form). Enrollment forms may also be obtained by contacting the Administration Office at the address or telephone number listed in the Plan booklet or at the Plan’s website.
Dependents will not be eligible for coverage until all requested enrollment documentation is provided. Please note: If requested enrollment documentation is not provided, the Plan will not pay claims until the documentation is received.
